Services are provided at no cost to individuals with Amerigroup, CareSource, or Peachstate/Cenpatico insurances.
Services are available to individuals with coverage through most major insurance companies. Eligibility and benefits are determined by the specific terms of your insurance plan.
If you do not have insurance, we offer a sliding scale fee structure based on household income to ensure services remain accessible.

Enrolling with Heritage Care Place is simple. You can either enroll yourself or refer someone else by completing our encrypted referral forms. Once submitted, our administrative team will review the referral for eligibility and notify you via email of the outcome.
